NOTICE OF CHANGE FORM and INSTRUCTIONS

The Notice of Change forms are intended to be used to report any major change in your business or residence that could have an impact on your tax deductions. If you buy a new truck, take out a new business loan, buy or sell a portion of your territory or buy or sell your home these things can all affect your business tax return and the information that DSA uses to prepare it. Whenever such an event takes place, simply fill out the Notice of Change form and mail it to DSA.

If you move or have a change in your Home Office Square footage please submit a Home Office Deduction Worksheet with your updated information.
